The answer usually lies in poor design or improper testing before release. If software is buggy, released too early without proper testing, and the users have a bad experience with it, no matter what you try, usually they will not use the software again, even if subsequent releases are solid and work well. Planning the testing for software is as much an art as it is technical, for there are not just go/no go or pass/fail results with software, but emotional responses and interaction criteria to manage as well.
